Job Summary and Qualifications
Provides psychosocial support to Cellular Therapy and Blood Cancer patients, caregivers, and families throughout the transplant continuum of care. Responsible for the management of continuous patient assessment, screening, and evaluation. Responsible for providing emotional support, counseling, advocacy, and the coordination and planning of discharge and transitional care. What you will do in this role: Optimize patient outcomes and quality of life throughout the blood cancer/cellular therapy trajectory. Maximizes patient and family psychosocial functioning and adaptation to diagnosis, comorbidities, and disease related outcomes. Maintains and promotes professional standards and development by participating with in-services, committees, staff meetings, and performance improvement activities. Assists in development of staff and team through educational guidance on professional boundaries, effective coping and communication strategies. Provides staff resourcing and support through debriefing sessions during critical time points. Participates in formal training in the management of patients receiving cellular therapy. Complies with all policies, procedures, and regulatory standards. Provides mental, emotional, and behavioral support to patient, caregiver, and family. Provides counseling for adjustment to illness, depression, anxiety, survivorship, grief, loss, advanced care planning and end of life decision making.
Assumes accountability and responsibility for the quality, continuity, and coordination of psychosocial care across the care continuum. Participates in discharge and transitional care planning in collaboration with the multidisciplinary team. Acts as patient advocate and system navigator. Facilitates cognitive, affective, and behavioral changes consistent with plan of care. Participates in activities and act as a leader in promoting best practices, department specific initiatives and process improvement. Assists patients and families with local and national support resources for both emotional and financial needs. Performs pre-transplant psychosocial evaluations, high-risk screening, and ongoing evaluation on all blood cancer/BMTpatients. Identifies psychosocial factors that may impact outcomes and develops clinical interventions based on needs. Provides, evaluates, and develops plan of care based on needs identified through the use of the Distress Screening Tool. Provides support to multiple patients (individual and family) either through assignment from care team and/or delegation from another member of the healthcare team. Evaluates the patient’s (individual and family) responses and outcomes to therapeutic interventions. Represents the organization positively within the community; participates in company-supported community/patient events and in-services Utilizes cost-effectiveness in evaluating care management patient needs. Promotes/practices cost effectiveness by the judicious use of resources (supplies and materials). Assess, intervenes, and identifies resources for potential patients that are either uninsured or underinsured Practices and adheres to the “Code of Conduct” philosophy and “Mission and Value Statement.” What qualifications you will need:
