We are currently seeking a PRN Medical Social Worker (MSW) to serve patients in the Orange, Osceola, Seminole and Brevard County. This role is vital to helping patients and families navigate the emotional, social, and practical challenges that may arise during the course of treatment and recovery. Key Responsibilities Conduct psychosocial assessments to determine patient needs and develop individualized care plans
Provide counseling and emotional support to patients and families
Assist with accessing community resources, financial assistance programs, and long-term care planning
Collaborate with the interdisciplinary healthcare team to ensure comprehensive patient care
Support discharge planning and coordination of services
Maintain accurate and timely documentation in compliance with agency and regulatory requirements Qualifications Master’s degree in social work (MSW) from an accredited institution
Current licensure in the state of Florida (LCSW)
Minimum of one year of experience in a medical or home health setting
Strong understanding of community resources and healthcare systems
Excellent communication, documentation, and organizational skills
Ability to work independently and manage a flexible schedule
