You will be an important part of our care team, helping patients and families navigate the emotional and logistical challenges of illness and recovery. Duties Conduct assessments to evaluate patients’ social, emotional, and environmental needs. Develop and implement individualized care plans in collaboration with healthcare professionals. Provide crisis intervention and support for patients facing acute challenges. Assist patients with disabilities in accessing necessary resources and services. Facilitate communication between patients, families, and healthcare providers to ensure continuity of care. Maintain accurate medical documentation in compliance with regulatory standards. Offer mental health counseling and addiction counseling as needed. Engage in case management activities to coordinate patient care across various services. Utilize diagnostic evaluation techniques to identify patient needs effectively. Collaborate with special education teams when necessary for pediatric patients.
