We’re now spreading the love through the country by opening our second location in Southwest Florida! We’re looking for a PRN Medical Social Worker to help us as we open our office on Metro Parkway. The ideal candidate would be interested or already has experience in connecting patients to local services based on their needs, background, and living situation in a home health setting. The starting rate is $45/visit. This position is expected to start in late June – mid July.
JOB DESCRIPTION
The ideal candidate for this role will be providing medical social services to patients in their homes based on physician orders and under the direction and supervision of the Director of Nursing or another appropriate supervisor. We prefer someone with experience delivering these services to elderly patients in a medical setting. Duties Assist in the admission process of the patient, to the Agency, by performing an initial evaluation, assessing the patient’s psychosocial status, and evaluating the patient, family, and home to identify socioeconomic, and emotional, factors that will affect the plan of treatment. Observe, record, and report changes in the patient’s emotional, and social factors, which affect the patient’s illness, and his/her need for care, and his/her response to treatment. Consult, with the attending physician, concerning alteration of the plan of treatment. Assist in development, and implementation, of the interdisciplinary patient care plan, as it pertains to medical social work Confirm, on a weekly basis, the scheduling of visits, with the Clinical Manager to facilitate coordination of other staff visits. Communicate with other team members and, when appropriate, instruct them in speech pathology techniques that they may use while working with the patient. Evaluate, teach, and counsel the patients and family’s response to, and effectiveness of, the medical social work intervention. Performs other duties as assigned. Qualifications Master’s Degree from a School of Social Work approved by the Council of Social Work Education Licensed or registered in the state of Florida (if a LCSW) Must have at least one year of healthcare experience Must have a criminal background check. Must have current CPR certification Valid driver’s license, dependable transportation, and valid automobile insurance Proficient with Microsoft Office products (Outlook, Word, Excel, PowerPoint). Excellent communication skills, both verbal and written, to effectively convey information. A commitment to providing outstanding patient care at all times.
