As a Social Worker at US Renal Care, you will be an integralpart of a cross-functional -team, working to help patients livingwith kidney disease achieve maximum social functioning andpsychological adjustment to dialysis treatment andrehabilitation. You will be part of an interdisciplinary team working to ensure-patients receive the best care, including conducting all requiredpatient assessment and -care planning activities such as assessingnew patient psychosocial needs and completing the KDQOL inaccordance with company policy and all state/CMS regulations. As anadvocate for your patients’ needs, you will coordinate communitiesof support for patients and their families, identify socialagencies and other resources (e.g., financial/funding), provideinformation and referrals, coordinate transient arrangements, andrepresent your patient as needed with appropriate local, state, andfederal agencies. As part of the interdisciplinary clinic team, promote teamwork,educate staff, and provide training around patient psychosocialcare. You will participate in all required continuing education andstaff meetings. You will collaborate with the Medical Director andphysicians and maintain positive relationships with area hospitals,agencies, vendors, and the community. Safety & Quality. You will help with clinical and operationalprocesses to improve patient health and minimize missed treatmentsand hospitalizations, achieving target goals for patient outcomes.
You will also participate in monthly Quality Assessment andPerformance Improvement (QAPI) activities and ensure compliancewith federal, state, and local laws and regulations. Master’s Degree in Social Work accredited by the Council of SocialWork Education (CSWE). Demonstrated working knowledge of the English language and abilityto communicate verbally and in writing. Basic computer skills, including Microsoft Office (Word, Excel,Outlook). Proficiency in all USRC clinical applications required within 90days of hire.
