Job Description

Are you a dedicated LCSW ready to make a real impact? We’re partnering with a leading behavioral health organization to find an experienced Licensed Clinical Social Worker to join their multidisciplinary team.

What You’ll Do:
  • Deliver individual and group psychotherapy to assigned clients
  • Engage clients actively in their treatment and recovery plans
  • Conduct outreach when client participation wanes
  • Coordinate care transitions during hospital or institutional stays
  • Complete Mental Health Rehabilitation (MHR) assessments and integrated summaries
  • Collaborate with the care team to develop and update individualized treatment plans
  • Ensure goals, objectives, and interventions align with clinical recommendations
  • Lead Service Planning Team meetings to review and adjust plans as needed
  • Accurately score and document

  • Partner with physicians and other professionals to optimize client outcomes
  • Educate clients and families on treatment strategies and promote active engagement
  • Attend all mandatory Department of Health and Hospitals (DHH) meetings
  • Maintain professional relationships with colleagues and community partners
What We’re Looking For:
  • Master’s in Social Work from an accredited program
  • Active LCSW license per R.S. 37:2701-2718
