Salary:
$25 – $34per hour A bit about us: We offer inpatient psychiatric care foradults and seniors with acute mental health needs. Our inpatientprograms are designed to provide short-term, acute care treatmentfor adults and seniors suffering from depression and other moodrelated disorders, psychosis, suicidal preoccupation, and any othermental health symptoms that present a major decline in functioning.
We have 4 inpatient hospitals located throughout Louisiana and eachfacility is staffed with a multidisciplinary treatment team.

Stabilization ofacute crisis conditions Comprehensive and multidisciplinaryevaluation Medication management for behavioral health and physicalneeds Our Hospital-Intensive Outpatient Program is a premierfull-service mental health program. Our highly trained, caringtherapy staff provides comprehensive and confidential treatment toadults and seniors suffering from depression, anxiety, bipolar,psychosis, changes in behaviors or other declines in functioningdue to a mental health condition. The IOP can also assist withadults and seniors that have a substance use issue which may becaused by their mental health condition. Our Hospital-IntensiveOutpatient Programs have 7 different locations throughout Louisianathat offer treatment to adults ages 18 and older. TreatmentProvided at our

QUALIFICATIONS
Experience in providingfor and managing the social service needs of the mentally ill toinclude by not limited to: – family, group and individualpsychotherapy with adults and/or geriatric patients – dischargeplanning and case management of inpatient psychiatric patients Mustbe licensed and/or certified to perform psychotherapy and/orcounseling within the State of operation. Must maintain currentprofessional licensure and demonstrates proof to Human Resourcesannually. Certification in Basic Life Support required.
Certification in non-violent crisis intervention.

SPECIFICRESPONSIBILITIES AND DUTIES
Responsible for providing for thesocial service needs of patients age 18 and older who are sufferingfrom mental illness. Provides clinical individual, group and familytherapy as ordered by the physician. Assesses and reassesses socialand emotional factors in order to help the patient and family copewith problems in daily living to help them follow medicalrecommendations to maximize the client’s health condition. Counselsand communicates to patients/families/primary caregivers issuesconcerning the facilitation of the medical plan of treatment anddischarge plan. Recognizes and utilizes community and familyresources to assist the client in long-term plans for life in thecommunity or to learn to live with the patient’s disability.
Develop the patient’s plan of care within required time frames andwith coordination of the other members of the multidisciplinaryclinical team (nursing, physician, other therapists, patient, andthe family) which addresses the psychosocial difficulties that mayinterfere with the health and wellness continuum. Begins thedischarge planning process with each patient upon admission.
Develops discharge plan in conjunction with multidisciplinaryclinical team, patient and family/caregiver. Organizes andmaintains a wide-range resource data set for the hospital to assistwith proper discharge planning for patients. Provides resourceinformation to other hospital personnel as needed. Develops,prepares and maintains individualized patient care progress recordswith accuracy, timeliness and according to policies, submitsaccurate documentation within mandated time frame. Keeps abreast ofsocial work trends and knowledge, obtains required annualcontinuing education inservices. Participates in the hospital’sQAPI program when directed by Director of Social services.
Completes psychosocial assessments on all newly assigned clientsand within time frames as mandated by hospital policy andregulatory standards. Attends and participates in weekly treatmentteam meetings and makes adjustments to treatment plan accordingly.
Ensures family contact, if applicable, is made with and documentedin 24 hours of admission. Serves as a liaison in the community asneeded. Adheres to hospital’s infection control and safetypolicies.
