Licensed Clinical Social Worker (LCSW/LICSW) – Case Manager (Utilization Review Focus)

Shift:

DAY, 40hr/week, 8hr/day Our client is seeking a compassionate, experienced, Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW) with strong utilization review experience to support their interdisciplinary team supporting children and young adults with developmental and complex medical needs. The ideal candidate will have a deep understanding of medical necessity criteria, managed care protocols, and clinical documentation review. This individual will play a critical role in assessing the appropriateness and efficiency of services, ensuring compliance with regulatory and payer requirements, and collaborating with providers to support high-quality, cost-effective care.

Minimum Entrance Requirements:

At least three years of Full time (or equivalent part-time) professional experience as a licensed social worker or active certification as a child protective worker as permitted by state law. LICSW or LCSW in the Commonwealth of Massachusetts, in good standing.

Key Responsibilities:

Coordinate, monitor, and implement individualized treatment plans for developmentally disabled patients in a hospital-based environment. Lead the interdisciplinary treatment team in the discharge planning process, with a focus on youth transitions to adulthood. Interview patients and families during pre-admission evaluations to assess suitability for admission and document findings for the Admissions Committee. Conduct psychosocial assessments for new admissions and maintain documentation within the medical record. Continuously review and analyze treatment data and outcomes to make recommendations and improve care planning effectiveness. Apply utilization review principles to assess medical necessity, ensure appropriate levels of care, and support cost-effective treatment planning. Collaborate with providers and state agencies (DDS, DCF, DPH, DESE, local school districts) to align services and ensure regulatory compliance. Provide patient and family education on treatment goals, available community services, insurance benefits, and transition planning. Explore and coordinate post-discharge resources, including housing, medical care, and educational or vocational programs. Facilitate unit-based education and skill-building sessions to promote patient independence and community readiness. Support efforts in data collection, quality improvement, and process enhancement related to case management and utilization management. Demonstrate sound clinical judgment, age-specific care knowledge, and empathy when engaging patients and families.

Preferred Qualifications:

Strong utilization review experience , including the ability to assess treatment plans against medical necessity criteria (eg, InterQual, Milliman). Experience working with youth and young adults in hospital or residential settings, especially those with developmental disabilities or complex behavioral needs. Familiarity with Massachusetts state agencies and service coordination across DDS, DCF, DESE, and DPH. Proven ability to interpret and apply psychosocial data, support transitions, and advocate for patients and families within the care system.
