Clinical Social Worker, Transplant (3 Days Left) Position Available In Suffolk, Massachusetts

Job Description:

Position Summary/Department Summary:

40-hour 100% onsite socialwork position to provide a range of clinical social work servicesand case management to patients in the lung, intestine and livertransplant program. This social worker will care for the organgroup population in all phases of transplant (evaluation,transplant admission and follow up).

Key Responsibilities:

• Psychosocial assessment as part of transplant candidacy for theprogram
• Counseling and crisis intervention
• Facilitation of theuse of hospital and community services
• Resource information andreferral, and program development
• Psychosocial consultation tomultidisciplinary team

Minimum Qualifications Education:

• Master’sdegree in social work

Experience:

• 2 years plus experience as alicensed Social Worker
• Experience in medical environment,bereavement, child protection issues preferred
• Flexibility andability to work in a fast-paced, dynamic environment
• Excellentcommunication skills are necessary.
