Medical Social Worker Position Available In Worcester, Massachusetts

Medical Social Worker Confidential Gardner, MA 01440 We are seeking a compassionate and dedicated Social Work Coordinator to join our team at a skilled nursing and rehabilitation center in Gardner, Massachusetts . As the Social Work Coordinator, you will play a key role in enhancing the quality of life of our residents by advocating for their psychosocial well-being, supporting families, and coordinating care plans with our interdisciplinary team.

Responsibilities:

Conduct psychosocial assessments and develop individualized care plans Support residents and families during admissions, care transitions, and discharge planning Provide emotional support and counseling to residents and families Participate in interdisciplinary meetings and contribute to resident care strategies Ensure compliance with state and federal regulations related to social services Serve as a liaison between residents, families, staff, and community resources

Requirements:

Bachelor’s or Master’s degree in Social Work (BSW or MSW) LSW or LICSW preferred (or eligible) Prior experience in a long-term care or healthcare setting highly preferred Strong communication, organizational, and advocacy skills Knowledge of discharge planning, Medicare/Medicaid, and local resources
