Position Summary:
The LTSS Service Care Manager is responsible for managing a caseload of healthcare members with long-term care needs. This position requires a high level of autonomy and fieldwork, conducting in-home visits, assessments, and ongoing care coordination. The role ensures members receive timely, cost-effective, and comprehensive services, while supporting their choice to remain in the least restrictive environment possible.
Key Responsibilities:
Complete in-person assessments with members, caregivers, or providers to obtain clinical and psychosocial information. Develop individualized care plans tailored to each member’s needs and goals. Monitor service delivery and member progress through both telephonic and in-person follow-ups. Authorize, coordinate, and track services and referrals to ensure no gaps in care. Collaborate with healthcare providers, discharge planners, and interdisciplinary teams to coordinate services across care settings. Support members with appeals, grievances, and navigation of long-term services and supports. Maintain accurate and timely documentation in electronic health records. Comply with state, federal, and Centene regulations and standards.

Experience:
Minimum 2 years of care management or case management experience Background in physical health, home health, utilization management, or discharge planning
Technology Proficiency:
Experience with Microsoft Office and electronic medical records (EMR)
Preferred Qualifications:
4-6+ years of total care management experience Experience in long-term care, home health, or discharge planning Familiarity with Medicaid or managed care organizations Strong documentation, time management, and organizational skills
