Your Impact as Medical Social Worker You’ll be at the heart of our integrated care model, bridging behavioral health and medical services in our HIV and primary care clinics. Core Responsibilities Patient-Centered Care & Support Conduct comprehensive psychosocial assessments addressing HIV care, gender-affirming services, mental health, substance use, housing, and overall well-being Navigate complex healthcare systems alongside patients, facilitating seamless access to medical, behavioral health, and community resources Provide crisis intervention and individual counseling (18 scheduled hours, 14 seen weekly) Lead group counseling and education on HIV diagnosis, safer sex practices, harm reduction, and gender-affirming care Ensure medication access through timely lab coordination, medication planning, and patient education Program Administration & Development Support Ryan White HIV Program administration, including data collection, reporting, and federal compliance Collaborate on research initiatives and grant writing to secure funding for HIV and gender-affirming care programs Act as liaison between clients, providers, pharmacy, and community partners for truly integrated care Provide supervision and mentorship to macro-social work students Community Engagement & Advocacy Partner with community organizations to reduce stigma and promote health equity Design and facilitate educational workshops and support groups Develop outreach strategies to connect marginalized populations with essential services Champion peer support and health education programming
What You Bring Required Qualifications:
✅ Licensed Clinical Social Worker (LCSW) in NC or eligible for licensure within 3 months ✅ 2+ years experience in medical or community health settings ✅ HIV care or gender-affirming services experience preferred ✅ Crisis intervention and therapy expertise (short and long-term) ✅ Ryan White HIV Program knowledge and federal guidelines familiarity ✅ Electronic health records proficiency and clinical documentation compliance ✅ Workshop and support group facilitation experience
Preferred Qualifications:
Bilingual in English and Spanish Trauma-informed care expertise HIV Care Continuum and PrEP knowledge North Carolina HIV Medication Assistance Program (HMAP) familiarity Student supervision experience
