The Clinical Social Worker II provides social work services topatients and their families to assist in meeting psychosocial andclinical needs. Using expertise and experience, this roleidentifies and serves the needs of patients, both inside andoutside the clinical setting utilizing therapeutic and otherclinical techniques.
Talent Pool:
Allied Health Determines which patients to focus clinical social work efforts onto help staff and patients alike be in the correct environment withappropriate social work support. Analyzes information from both the patient and clinical staff tounderstand the patient’s situation and is able to communicate thisinformation to all parties to better prevent issues that arise frominformation gaps. Links patients with community resources and community healthopportunities based on clinical knowledge and experience to ensurethat patients get the highest quality of necessary care. Crafts a care plan for patients with minimal assistance fromclinical staff to enable the patient to get the treatment that isright for them. Educates staff, patients and families alike about criticalinformation related to care to help all parties involved betterunderstand the situation, serving as a liaison. Provides crisis support and leadership in high-tension situationsfor staff, family and patients using clinical expertise to ease allparties in high-tension situations, giving explanations that helpde-escalate the worries of patients and their families. Provides clinical work services to both patients and their familiesto meet current and future psychosocial and medical needs. Completes psychosocial assessment for patients to understand theirsituation and be able to link them with the option of care thatmakes the most sense for the patient’s medical and socialstatus. Utilizes various counseling and therapy techniques to help placethe patient and their family at ease with regards to clinical andmedical work. Performs other duties as assigned.
