HOSPICEMEDICAL SOCIAL WORKER
Per Diem Job Description:
Provide directpatient care services and documentation of patient care data.
Provide clinical assessment for the development of individualizedcare plan, and set goals for patient treatment. Apply a workingknowledge of psycho-social principles and family dynamicdevelopment. Identification and assessment of social, emotional,financial and environmental factors which may affect the medicalplan of treatment and desired outcome. Communicate with agencypersonnel and coordinate with other services in providing optimumcare. Provide assistance with homecare visits as needed. Relatewell to patient/family at all levels responding professionally toverbal and nonverbal communicators. Utilize community resourceplanning in providing patient services, and educate patients/familyon how to utilize these resources. Evaluate bereavement needs offamilies through initial bereavement assessments andinterdisciplinary team coordination. Provide bereavement supportand grief care to patients and caregivers.
Job Requirements:
Masters of Social Work degree from a school accredited by theCouncil on Social Work Education with a minimum of one yearpractical experience, preferably in the home health/hospice careenvironment. Strong interpersonal skills, with the ability toeffectively communicate within a multidisciplinary setting. Accepttravel and working conditions as assigned.
