Job Description

Carolina Caring is seeking a Master of Social Work to fulfill to provide case management and counseling services to the patient, family and/or caregiver. The medical social worker provides psychosocial assessments and develops a corresponding plan of care as coordinated with the Interdisciplinary Team. This social worker will see home patients in the Mecklenburg and Cabarrus counties. This position is Monday-Friday, 8:30am-5:00pm Full-time positions offer an excellent benefit package and paid time off and mileage reimbursement.
