HOSPICE MEDICAL SOCIAL WORKER
Full Time, Days Job Description:
Provide direct patient care services and documentation of patientcare data. Provide clinical assessment for the development ofindividualized care plan, and set goals for patient treatment.
Apply a working knowledge of psycho-social principles and familydynamic development. Identification and assessment of social,emotional, financial and environmental factors which may affect themedical plan of treatment and desired outcome. Communicate withagency personnel and coordinate with other services in providingoptimum care. Provide assistance with homecare visits as needed.
Relate well to patient/family at all levels respondingprofessionally to verbal and nonverbal communicators. Utilizecommunity resource planning in providing patient services, andeducate patients/family on how to utilize these resources. Evaluatebereavement needs of families through initial bereavementassessments and interdisciplinary team coordination. Providebereavement support and grief care to patients and caregivers.
JobRequirements:
Masters of Social Work degree from a schoolaccredited by the Council on Social Work Education with a minimumof one year practical experience, preferably in the homehealth/hospice care environment. Strong interpersonal skills, withthe ability to effectively communicate within a multidisciplinarysetting. Accept travel and working conditions as assigned.
