Job Description
Ability to identify “high risk” patients in order to provide social work services and complex behavior health and substance abuse and discharge planning needs. Experience interviewing patients, families and/or community support services to assess patients’ psychological, social, environmental, and financial status and meet identified needs. Experience educating patients and/or families regarding discharge planning, negotiating, coordinating and advocating community based services (DYFS, APS, Office of Public Guardian, DDD, etc.).
Requirements:
Masters Degree in Social Work from professional school accredited by the Council on Social Work. Min. 3 years current relevant social work experience. Substance Abuse Specialty preferred, knowledge of telehealth technology desires. Bilingual English/Spanish a plus.
Current NJ Social Worker License:
LCSW required
