Licensed Clinical Social Worker (LCSW) Position Available In Nassau, New York

Licensed Clinical Social Worker (LCSW) Visiting Counselors of New York, LCSW, PLLC Bellmore, NY We are seeking a highly motivated and experienced Licensed Clinical Social Worker (LCSW) to provide in-home counseling and remote counseling services to older adults in Bellmore and the surrounding area. The LCSW will be responsible for conducting assessments, developing treatment plans, and providing individual therapy to clients in their homes and/or remotely.

Key Responsibilities:

Conduct comprehensive assessments of clients’ social, emotional, and psychological needs. Develop individualized treatment plans and set goals in collaboration with clients. Provide individual therapy services to individuals in their home and remotely. Monitor clients’ progress and make adjustments to treatment plans as needed. When necessary provide clients with community linkage to help address other needs.

Qualifications:
