JOB TITLE

Licensed Clinical Social Worker

REPORTS TO

: Director of Behavioral Health The following statements reflect the general duties, responsibilities and competencies considered necessary to perform the essential functions of the job and should not be considered as a detailed description of all the work requirements of this position.

RESPONSIBILITIES

: Engages patient(s) in therapeutic process through one-on-one therapy and/or group therapy sessions Provides therapeutic intervention to patients with behavioral health needs, including individual, family, and group psychotherapy, community work, crisis intervention, case management, other treatment modalities Prepares psychosocial histories/reports and maintains treatment records, in accordance with clinic and regulatory guidelines Conducts psychosocial assessments and develops and implements treatment plans Obtains collateral information from patients, family members, outside treatment providers, schools, and other relevant team in the assessment of medical and behavioral health needs Responds to referrals and consultation requests from primary care providers Participates in agency Quality Assurance activities and case discussion meetings Provides task supervision and consultation to social work interns, other clinical trainees, and volunteers if needed Other duties as assigned.

QUALIFICATIONS

Master’s Degree in Social Work from an accredited school of social work New York State License required Strong verbal and written communication skills Previous work experience in behavioral health or health setting preferred Ability to travel to other sites as needed Bilingual in English and Spanish/Creole required.

SALARY :

Commensurate with experience
