Monday-Friday, Hours of operation range from 8 am-7:00 pm . Thereis also Call involved for off shift and weekend. Provides Crisisintervention to SSMH and GGH, and Counseling for JDK OutpatientClinic The Clinical Primary Therapist works with clients, eitherindividually or in groups, to address mental health concerns. TheClinical Primary Therapist applies knowledge and skill to assistclients in recovery and illness management. The Primary Therapistis responsible to establish the boundaries of therapy including acomfortable safe environment, caring therapeutic culture andfoundation for client / family engagement in recovery andbehavioral management. The Primary Therapist completes acomprehensive assessment gathering data and information resultingin clinical formulation as the framework for development of thetreatment plan. In collaboration with the client / family theTherapist establishes goals for treatment and identifies theinterventions to promote goal achievement. The Clinical PrimaryTherapist is responsible for timely accurate documentation in theclient s medical record, coordination of care with colleagues inthe clinic and colleagues throughout the continuum to secure theappropriate treatment resources to promote client health. ThePrimary Clinical Primary Therapist participates in the on callrotation to the Emergency Departments of Finger Lakes Health toprovide mental health crisis evaluationsComprehensive assessment,includes the psychosocial history, lethality (risk of harm to selfor others), behavioral observations, substance dependence /abuse,the client’s chief complaint, focus/goals of needed treatment,physical, developmental, cognitive, mental and emotional healthstatus, demographic profiles, history of health patterns,illnesses, past history and treatments, and family, social,cultural, community supports and strengths that can be used topromote health. Assessment includes screening for co-occurringsubstance use disorder. The assessment results in a clinicalformulation which informs the treatment plan Reassessment ofbehaviors is completed at each visit. Diagnosis and potentialproblem statements are derived from assessment data. Diagnosis andrisk factors are discussed with the patient and family. Treatmentplan are person centered, recovery oriented and responsive tocultural and linguistic needs of the client. Treatment plangoals/objectives are clearly linked to the assessment and dischargecriteria, are individualized and person centered Treatment plansreflect attainable steps toward goal achievement and include targetdates, contain specific evidence based interventions/services usedby the therapist and identifies frequency of services. Treatmentplans are developed signed by client and therapist within 30 daysof admission. Discharge criteria are identified within thetreatment plan, updates and reflect the desired accomplishments ofthe client. Completes and maintains clinic records and progressnotes according to regulations and within specified timeframes.

Progress notes are linked to goals/objectives and summarizeservices provided, interventions utilized and clients response andprogress toward goals. New information impacting treatment isincorporated in the documentation and considered in the treatmentplan updates. Evidence of engagement and retention is reflected inthe documentation t. Discharge criteria are developed and documentsexpected change in patient statusEvidenced based interventionsreinforce healthy behaviors and interactions. Counseling promotesthe patient’s personal and social integration and includes healtheducation in the areas of increased coping, medication use, socialskills, and symptom managementSystematically evaluates quality andeffectiveness of Therapy Practice both individual and group, anduses the results of quality improvement activities as well asevidence based interventions to initiate changes in individualtherapy practice and to influence delivery of mental healthservices within the clinic. Engages in self-evaluation of practice,identifying strengths and areas in which professional developmentwould be beneficial and takes accountability / initiative toincrease knowledge and enhance skill. Collaborates with members ofthe clinic team to achieve productivity standards, reduce no showrates and facilitate client engagement.
