Job Title & RoleDescription:

The Licensed Clinical Social Worker (LCSW) at UpwardHealth plays a crucial role in assessing and addressing thepsychosocial needs of patients. As a member of theinterdisciplinary care team (IDT), the LCSW collaborates with teammembers to develop comprehensive care plans that address both themedical and emotional well-being of patients. This positioninvolves applying behavioral intervention techniques such asMotivational Interviewing, trauma-informed care, and person-firstthinking. The LCSW also works to connect patients and theirfamilies to community resources, facilitates transitions of care,provides short-term, risk-focused care management, and intervenesduring crises when necessary. This role ensures that patientsreceive comprehensive care while promoting patient self-managementand autonomy.

Skills Required:
  • Licensed Clinical Social Worker(LCSW) with all state certification and licensing requirements met
  • Minimum of three (3) years of social work experience, with atleast two (2) years in a healthcare setting (e.g., physician’soffice, PCMH, hospital, clinic, home care, or nursing home).
  • Knowledge of Medicaid and Medicare, as well as managed careorganizations.
  • Strong communication skills, both oral andwritten, with the ability to collaborate effectively withmultidisciplinary teams.
  • Experience in home care or field casemanagement is preferred.
  • Working knowledge of local and stateresources to support patient needs.
  • Ability to assess and addressSocial Determinants of Health (SDOH).
  • Ability to manage patientcrises and provide focused care interventions.
  • Valid statedriver’s license and access to an automobile.
