Job Description

AnMed Health Social Worker – Medical Group in Anderson
, South Carolina AnMed Pediatrics – Fant Street is seeking a licensed social worker to integrate into a pediatric practice and assist with the care management of adolescent patients ages 11-17 with ADHD, anxiety, and depression. In addition, the Social Worker will assist with care management of medically complex children. Duties include planning for the care management of targeted populations, maintaining knowledge of community agencies and community resources for referrals and care partnerships, interviewing, assessing needs, and referring patients/families to resources for social, emotional, or financial assistance when appropriate, and serving as a patient advocate to help in the decision­-making process. In addition, the social worker will assist the patient and/ or family with applying for financial assistance, services, or counseling to the appropriate community agency or other source of help and p rovides in-home and other nontraditional visits as needed.

EDUCATION/QUALIFICATIONS

Required Qualifications include: Bachelor’s degree in social work required, Masters preferred or related health care field. One year of hospital/healthcare related community experience required. SC License Bachelor of Social work-LBSW required. Knowledge of how to navigate community resources is required. Excellent communication skills. Works in collaborative team approach. Must have current South Carolina motor vehicle license with reliable transportation. Willing to use personal transportation (mileage reimbursed) to do home visits and other patient-centered travel such as joint physician visits. Comfortable in a variety of settings meeting patients and working with a team. Familiarity with EHR, Microsoft Word and/or other similar computer software. Willingness to learn new skills.

Preferred Qualifications include:

Two years of hospital/healthcare related community experience. Certified Health Coach.
