POSITION SUMMARY
This position requires management and assistancefor patients of Williamson Medical Center, providing for short-termcounseling/crisis management for patients and their families, whoare impacted by adjustment to illness or diagnosis, change inresidence or lifestyle, and/or financial concerns which are aresult of their illness and hospitalization. This counseling/crisismanagement includes assistance with psycho-social, alcohol and drugproblem assessment and management, discharge planning withappropriate referrals, education for patients/families and theappropriate use of resources. The Social Worker activelyparticipates in consistent collaboration with physicians, staff,patients, families, and community agencies in the dischargeplanning process. This position requires that early dischargeplanning occurs, communicating effectively and the appropriatedevelopment of a discharge plan; therefore, alleviating/reducinganxiety for patients and their families. This role works within theknowledge and regulations related to CMS, Commercial Payers andBundle programs in delivering appropriate discharge planning.
POSITION REQUIREMENT
Formal Education /
Training:
Requireslicensure by Tennessee Health Related Boards in Social Work. LCSWpreferred Earned a Master’s degree in social worker or a comparableMaster’s level degree from a college or university.

Hospital or Clinical setting required.

Computer-Internet, Meditech and Allscripts CareManagement, telephone, printer, fax, copier, language line.
GoodVerbal/Written Communication Skills Physical Environment:

Required Weekend Rotation PhysicalEffort:
Must be able to walk short distances in hospitalenvironment and patient rooms and possible manual lifting 10-15lbs, Visual and hearing acuity.
PERFORMANCE STANDARDS
Providespsychosocial/crisis/ETOH/drug problem assessment, short termcounseling and discharge planning to hospital and ER patients whorequire inpatient/outpatient treatment within 24 hours or nextbusiness day of requests. Identifies resources by specialty andinsurance contracts and appropriately documents inMeditech/Allscripts regarding assessment of patient’s level offunctioning, psychosocial and discharge needs. Consistently setspriorities and exhibits efficient time management skills withassigned workload, responding to referrals within 24 hours or nextbusiness day of requests with documentation in Meditech within theconfinements of scheduled shift. Screens patients who do not haveconsults to identify discharge needs and for the need of postdischarge referrals, with 48 hours of admission followed byappropriate documentation in Meditech Responds to referrals within24 hours or next business day of requests with documentation inMeditech Completes assessments within 24 hours or next business daydocumenting in Meditech/Allscripts addressing the patient’s levelof functioning, psychosocial and discharge needs communicatingclearly accurate information to patient/family. Collaborates withmembers of the healthcare team, patient/family in the developmentof an appropriate discharge plan, providing the appropriateresources, post discharge option lists, and completion of a postdischarge preference form when services are determined. Initiatesprecertification for those patients needing authorization for postdischarge care/transportation. Maintains positive workingrelationships with community agencies, physicians and other membersof the healthcare team by communicating clearly timely and accurateinformation. Consistent timely delivery of all appropriate andupdated patient information to post discharge providers inAllscripts. Communicates Bundle program information to selectedpatients with the delivery of CMS Bundle letter within 24 hours ornext business day of admission. Assists Case Manager in patientconversation and delivery of information related to dischargeappeals, and payer denials. Actively participates in DischargeAppeal process. Enters discharges with correct dischargedisposition in Meditech within 3 days post discharge.
