Job Summary Provides quality,cost-effective care for patient/families and/or others in targetpopulation. Evaluates patients/families at Hartford Hospital’s 24hrEmergency Department to determine Psychiatric/Substance Abuse careneeds. Assures access, provides assessment and/or direct care,coordinates required resources and partners with receiving programsand clinicians. By utilizing excellent communication andnegotiation skills, focuses on providing the appropriate service inthe most cost-effective formula to assure that the patient,provider, payor and health care organization’s goals are met.
Reports to Unit Manager/Program Manager. Qualifications
- LCSW,PhD, or PsyD licensed to practice in the state of CT.
- Must meetrequirements for internal credentialing.
- Experience inassessment, triage and intake preferred. Individual programs mayrequire specific additional certifications.
- Demonstratescompetencies, including knowledge and behaviors, to meetage-specific patient needs.
- Knowledge of community mental healthresources; understanding of addiction and recovery process; andgroup counseling skills and individual assessment skills arerequired.
- Excellent interpersonal/communication skills required.
Bilingual Spanish/English is a plus.
- Basic computer skills andknowledge of medical terminology preferred. We take great care ofcareers.
