Job Responsibilities Direct PatientCare

  • Conducts emergency psychiatric assessments. Interprets thepsychiatric and psychosocial needs of the patients and gatherscollateral information to determine appropriate disposition.
  • Conducts advocacy, referral and discharge planning to meetpatient/family needs. Works with patients’ community behavioralhealth and medical providers to assure coordination of clinicalcare.
  • Facilitates treatment process by referring patient/familymembers to appropriate level of care, and other resources as wellas providing direct crisis intervention services.
  • Coordinatescare and dispositions of psychiatric patients with EmergencyDepartment staff while working effectively as a team member bothwithin and across clinical setting to provide an integrateddelivery of health care services through communication, cooperationand collaboration.
  • Documents assessments and all case managementactivities related to patients.
  • Initiates treatment plan forlonger length of stay patients in collaboration with provider.

Clinical Experience

  • Actively participates in quality and safetyinitiatives to support the unit which may include participation ininitiatives, committees, workgroups, and aftercare calls.
  • Participates in administrative and development activities includingdebriefings, team meetings, clinical supervision of staff and staffeducation.
  • Works as a team member to contribute to improvedpatient experience.
  • Ensures the cost-effective utilization ofresources. Qualifications
  • Master’s Degree in Social Work,Professional Counseling, or Marriage and Family Therapy
  • A minimumof two years’ experience in a psychiatric setting required,preferably in an emergency department/crisis role
  • Four years’experience in a psychiatric setting, preferably in an emergencydepartment/crisis role is preferred
  • Current CT license required:

LCSW, LMFT, or LPC

  • Computer proficient
  • Ability to assessproblems, determine solution and make decisions within a timelymanner
  • Utilize supervision and team approaches in the context ofpatient assessment and clinical interventions
  • Excellentcommunication skills, with the ability to establish relationshipswithin and outside their entity
  • Demonstrates competencies,including knowledge and behaviors, to meet age specific patientneeds
  • Focused on personal, patient/client and unit/staff safety
  • Assumes responsibility for self-improvement by identifying,communicating and seeking resources or instruction/validation tomeet own learning needs, and is self-directed in maintainingcurrency of validations, certifications, licensure and training
  • Is cognizant of health-related social needs and culturalcompetence. Recognizes and addresses healthdisparities-inequalities and injustices directed toward clients,related to access to care and provision of health services
  • Develops and maintains cultural competence with an understanding ofthe history, traditions, values, and family systems of clientgroups as relates to health care and decision-making.
