Job Summary Works incoordination with interdisciplinary teams to organize, prepare, anddeliver evidence based clinical services across the continuum ofcare. Maintains ethical principles and professional standards ofpractice in adherence to the discipline’s professional code ofethics. Job Responsibilities 1. Conducts a comprehensivebio-psychosocial assessment and establishes a care plan, asappropriate. 1. Collaborates with the interdisciplinary Team toassess for appropriate level of care and risk factors; develops andimplements care/discharge plan and coordinates consults/referralsto remove barriers and promote optimal clinical and financialoutcomes. 2. Utilizes specific therapeuticmethods/strategies/interventions in accordance with individualizedcare plan. 3. Monitors progress of interventions and revises planto meet patient/family needs. 4. Provides education topatient/family with respect to meeting established goals includingbut not limited to disease prevention, impact of illness anddisease progression, advocacy for benefits, health maintenance, andadherence to treatment regimens. 3. Provides direct patient carethat may include: case management, care coordination,interdisciplinary collaboration for care transition needs, crisisintervention/safety planning, counseling, and emotional support topatients and families coping with illness, trauma, or end-of-lifecare, referral and discharge planning. 4. Refers patient toappropriate levels of care and available resources that promote theindividualized care plan. 5. Maintains timely patient-centereddocumentation that reflects the patient and client systems’pertinent information for assessment, interventions and outcomes inaccordance with regulatory licensing requirements and third-partyagencies. 6. Independently able to provide clinical social workinterventions to all patient populations. 7. Participates inadministrative and development activities including team meetings,clinical supervision of staff and staff education. 8. Providesocial work field supervision to second year social work students.9. Supports the development of hospital and/or unit-basedprogramming to meet the clinical and strategic needs of thepatients and the milieu. 10. Works as a team member to contributeto improved patient experience. 11. Ensures the cost-effectiveutilization of resources. Qualifications

  • Master’s Degree inSocial Work, or Counseling, or Marriage and Family Therapy orRelated Human Service Field
  • Previous experience in a healthcaresetting
  • LCSW, LPC, LMFT, LADC
  • Valid Driver’s License
  • Computerproficient
  • Ability to assess problems, determine solutions andmake decisions within a timely manner
  • Utilize supervision andteam approaches in the context of patient assessment and clinicalinterventions
  • Excellent communication skills, with the ability toestablish relationships within and outside their entity
  • Demonstrates competencies, including knowledge and behaviors, tomeet age specific patient needs.
  • Focused on personal,patient/client and unit/staff safety
  • Assumes responsibility forself-improvement by identifying, communicating and seekingresources or instruction/validation to meet own learning needs, andis self-directed in maintaining currency of validations,certifications, licensure and training.
  • Is cognizant ofhealth-related social needs and cultural competence. Recognizes andaddresses health disparities-inequalities and injustices directedtoward clients, related to access to care and provision of healthservices.
  • Develops and maintains cultural competence with anunderstanding of the history, traditions, values, and familysystems of client groups as relates to health care anddecision-making.
