Together, we can make a profound difference in the lives of those we serve! Responsibilities As a Counselor, you’ll be the vital link between our clinical team and direct care staff. You’ll conduct individual counseling sessions, facilitate engaging skills-based and psycho-educational groups, and oversee the lively milieu management. Your role will be essential in coordinating our stage system, helping clients progress through their treatment journey, and modeling effective interventions. Get ready to inspire and empower!

What You’ll Do:
Individual & Group Sessions:

Facilitate weekly individual counseling and skills-based, psycho-educational group sessions with adolescents and young adults, all within port Healthcare’s clinical model.

Milieu Management Maven:

Be a consistent and supportive presence in the milieu, helping Care Coordinators (CCs) manage clients and set healthy limits while collaborating with residential staff.

Progress Evaluator:

Evaluate reflective assignments and make advancement recommendations based on clients’ progress through the stage system.

Activity Scheduler:

Coordinate experiential and recreational activities tied to earned privileges, ensuring treatment is engaging and enjoyable!

Goal Setter:

Develop treatment goals and coordinate services in collaboration with the client’s treatment team, informed by intake assessments and regular 30-day treatment plan reviews.

Crisis Management Pro:

Conduct risk or crisis assessments and provide timely crisis intervention and management as needed.

Educator Extraordinaire:

Provide substance use education to clients, families, and the community, helping to spread awareness and understanding.

Documentation Dynamo:

Complete required documentation (assessments, treatment plans, progress notes, discharge summaries) promptly and according to established standards.

Service Coordinator:

Offer case management and discharge planning support to ensure a smooth transition for clients.

Team Player:

Attend weekly treatment team meetings, staff meetings, and clinical supervision to stay connected with your colleagues.

Action Planner:

Distribute weekly action plans for each client to staff, keeping everyone informed and aligned.

Community Ambassador:

Attend meetings, trainings, and conferences representing port Healthcare as needed.

Other Adventures:

Perform other duties as assigned-because every day brings new opportunities! Qualifications Master’s Degree in counseling, psychology, social work, or related field preferred.
Credentialed (CDP, CAC, CADC, RAD, SUDC, etc.) as a mental health or substance abuse counselor preferred. A willingness to pursue certification in the state in which services are provided is required.
Prefer board registered (APCC, LAPC, ASW, AMFT, MHCA, LMHC) with credentialing agency within the state where services are provided.
Experience working with individual, and/or group therapy with young adults/adolescents required.
Experience with mental health, substance abuse, or trauma-informed treatment preferred.
