Job Summary:

TheLicensed Behavioral Health Clinician works in coordination withinterdisciplinary teams to organize, prepare, and deliver evidencebased clinical services across the continuum of care.

Keyresponsibilities:

• Conducts a comprehensive bio-psychosocialassessment and establishes a care plan, as appropriate withinEmergency Department.
• Provides direct patient care that mayinclude: case management, care coordination, interdisciplinarycollaboration for care transition needs, crisis intervention/safetyplanning, counseling, and emotional support to patients andfamilies coping with illness, trauma, or end
• of-life care,referral and discharge planning.
• Refers patient to appropriatelevels of care and available resources that promote theindividualized care plan.
• Maintains timely patient-centereddocumentation that reflect the patient and client systems’pertinent information for assessment, interventions and outcomes inaccordance with regulatory licensing requirements and third-partyagencies.
• Works as a team member to contribute to improvedpatient experience.
• Provide social work field supervision tosecond year social work students NO weekends, NO holidays! Monday-Friday; 3:00PM-11:30PM. Behavioral Health Clinicians are eligiblefor a Sign-On Bonus in accordance with program guidelinesQualifications
• Current CT license LCSW, LMFT, or LPC required.

LCSW is preferred.

• Master’s degree in Social Work, ProfessionalCounseling, or Marriage and Family Therapy required
• A minimum oftwo years’ experience in a psychiatric setting required, preferablyin an emergency department/crisis role, four years’ experiencepreferred.
• Highly effective verbal and written communicationskills, Bi-lingual Spanish language skills preferred.
• Iscognizant of health-related social needs and cultural competence.
