M-F:
3 days: 8:30am
- 5pm and 2 days
- 11:30
- 8pm, two days per week.
Occasional weekend or evening events Job Summary Implements the established bereavement support services and protocols through client assessment, goal setting, plan of care implementation, progress evaluation, documentation, advocacy, and coordination of service delivery until client goal resolution is achieved. Clients are defined as adults and children. Provides short term bereavement counseling through emotional and psychosocial support and services to assist with issues related to grief, loss, and adjustment.
Qualifications:
Master’s degree in social work or related field required. One year experience or education in grief support and mental health. Hospice experience preferred. Specific experience in action and behavior focused group facilitation, individual support, family systems, geriatrics, human development and behavior, facilitating adjustment to loss, catastrophic illness and suicide risk assessment. Includes knowledge of child and adolecent development. Competence with the technical components necessary to complete the job.
