Role:
The Bereavement Counselor provides a range of bereavement services to bereaved survivors.
Key Responsibilities:
- Provide individual and group counseling and therapy for Hospice patients, family members andmunity individuals who have experienced the loss of a significant person through grief.
- Demonstrate contact of all clients within established time frames and provide appropriate documentation. Document at least two additional attempts to reach an assigned client before closing the case.
- Demonstrate a willingness and ability to manage tasks and case loads assigned, from any departmental program, in an effective, productive and timely manner, meeting the standards that reflect expected productivity.
- Demonstrate a willingness to provide counseling services such as on call crisis intervention, individual and group, on a flexible schedule as needed, including some week nights, weekends and at camp.
- Document all client contacts and care plans accurately according to department andanization policy in a timely manner.
This includes establishing and maintaining all legally required files according to state, law, federal law andanization policy. As demonstrated by chart audits.
Knowledge & Skills:
- Excellentmunication; via phone, email, and in person.
- Attention to detail & ability to handle confidential information following HIPPA guidelines.
- Demonstrate multi-tasking andanizational skills.
- Must be flexible to work in other locations if needed.
Education/Training Requirements
If hired after 5/26/2024, master’s degree in counseling, social work, marriage and family therapy, or related field required.
If hired prior to 5/26/2024, bachelor’s degree in counseling, social work, marriage and family therapy, or related field preferred.
Clear Level 2 background check
Certificates/Licenses/Registration
Must possess a valid Drivers’ License, current auto insurance, and reliable transportation.
Professional Licensure as required by state w care is being provided.

Demonstrate a knowledge, understanding andpliance with the standards for counselors in clinical practice.
Demonstrate a willingness and ability to manage tasks and case loads assigned, from any departmental program, in an effective, productive and timely manner, meeting the standards that reflect expected productivity.
Demonstrate a willingness to provide counseling services such as on call crisis intervention, individual and group, on a flexible schedule as needed, including some week nights, weekends and at camp.

Communicate, collaborate and consult with other departmental counselors, members of the Interdisciplinary Team, otheranization staff and external customers with integrity and professionalism to ensure the best possible services are given to all customers internal and external.
Attend and participate in department, staff and other meetings as requested by theanization and the Director. Make rmendations for the improvement of programs and activities within the department.
Represent thepany in a positive and professional manner at all times with others, e.g., co-workers, management, vendors, and customers.
Consistently represents Chapters Health System with integrity and professionalism.
Complete all job responsibilities in a thorough andplete manner, utilizing good judgment in all aspects of the job. Carry out duties and responsibilities in a manner that exemplifies excellent customer service.

Assist with special projects, as requested.
