General Overview:

The Mental Health Counselor will play a crucial role in providing mental health services to adults who are experiencing homelessness. As a member of our Care Services Team, the counselor will assess participants’ mental health needs, design personalized treatment plans, and offer therapeutic support to address their emotional and psychological challenges. The ideal candidate will demonstrate empathy, cultural competence, and a commitment to empowering vulnerable populations to achieve positive outcomes.

Responsibilities and Duties:

The functions of this position include:

Conduct Mental Health Assessments:

Conduct initial intake assessments to evaluate participants’ mental health needs and identify any co-occurring disorders or underlying mental health issues.

Develop Individualized Treatment Plans:

Collaborate with participants to design person-centered treatment plans that align with their goals, strengths, and challenges. These plans should address their unique circumstances and foster personal growth and resilience.

Provide Individual Counseling:

Offer one-on-one counseling sessions to participants, utilizing evidence-based therapeutic techniques to support emotional well-being, stress management, and personal development.

Crisis Intervention:

Be prepared to provide crisis intervention and de-escalation techniques when necessary, ensuring the safety and well-being of youth experiencing acute mental health challenges.

Group Counseling:

Facilitate group therapy sessions, creating a safe space for youth to share experiences, build social connections, and develop coping skills. Collaborate with

Interdisciplinary Team:

Work closely with other teams, such as care managers, spiritual care, medical partners, and outreach services team, to ensure a coordinated and holistic approach to client care.

Monitor Progress and Outcomes:

Regularly evaluate clients’ progress and adjust treatment plans to achieve positive outcomes and address evolving needs.

Advocacy and Referral:

Advocate for participants’ needs to access additional community resources and support services and enhance their overall well-being and stability.

Maintain Records:

Maintain accurate and confidential client records, documenting all interactions, progress notes, and treatment plans in compliance with organizational policies and legal requirements.

Professional Development:

Stay informed about the latest developments in mental health care, attend training sessions and workshops to enhance counseling skills, and maintain appropriate licensure or certifications.

Qualifications:

Requirements Master’s degree in Counseling, Social Work, Psychology, or a related field. Active state licensure or certification as a Mental Health Counselor (e.g.

LMHC, LPC, LMFT, LCSW

) is required. Experience working with homeless or vulnerable populations preferred. Experience in diagnosing and treating different Mental Health Disorders Knowledge of trauma-informed care, crisis intervention, and evidence-based therapeutic modalities. Strong interpersonal skills with the ability to build rapport and trust with clients from diverse backgrounds. Empathy, patience, and a non-judgmental approach to support clients facing mental health challenges. Excellent communication and collaboration skills to work effectively within a multidisciplinary team. Adherence to ethical guidelines and commitment to maintaining client confidentiality.
