JOB DESCRIPTION

Responsible for providing psychosocial services under the general direction of the Behavioral Health Program Coordinator in accordance with policies and protocols established by Morehouse Community Medical Centers and other regulatory agencies. The Behavioral Health Provider is responsible for implementing age appropriate educational programs, assessing psychosocial factors causing emotional and behavioral distress, determining appropriate methods of treatment to address these factors, and advocate for patient to motivate toward obtaining patient-determined goals of treatment. The Behavioral Health Provider implements interventions, methodologies, and functions in such areas as individual and group counseling, protective services and family consultations.

Some duties and responsibilities include:

Assess psychosocial and behavioral health needs, assess motivation toward change, assess challenges toward treatment success, treatment planning, and implementation of psychosocial services for patient while working to coordinate across multiple settings. Work with the patient’s health team members to provide needs assessment including: gathering and assessing information regarding the patient’s needs, mental status, support systems, and inform patient of available financial, community, and governmental resources. Work with patients’ family members as deemed appropriate in regard to confidentiality. Provide education content and counseling programs that will provide maximum benefit for each patient, both on an individual and group basis. Provide on-site evaluation of patients referred by other members of the health center, provide brief interventions in an acute crisis situation and coordinate referrals for any in-patient psychiatric needs. Counsel patients and interpret any specific psychosocial factors that are correlated to current behavioral health conditions, and advise on the incorporation of skills for the alleviation of anxieties and/or fears. Interpret nature of mental illness and prescribed course of treatment to patients and families. Appropriately communicates family issues to other staff members. Educate patient on patient’s partnership role in treatment including improvements to self-care practices, medication compliance, and compliance of treatment plan to assist with prevention of unnecessary ED or hospitalizations. Provide short-term counseling that is goal-oriented to meet specific behavioral health and psychosocial needs and explore available options. This may include identifying dependency issues, identity crisis, interpersonal communication barriers, negative relationship interactions, and a need to improve boundaries, etc.; involving family as necessary. Ensures confidentiality of interviews, treatment and documentation Work closely with all members of the health care team to develop a comprehensive treatment plan to meet the patient’s identified needs, revise treatment plan as needed, and drive the plan to insure optimal treatment benefit. Conduct surveys and studies relating to psychosocial status and patterns of population groups as directed. Prepare psychosocial educational materials as needed or required and conduct behavioral health related educational presentations to clinical staff, patients, and patient families. Coordinate with the medical staff in an integrated health team approach for the provision of services to patients. Provide referrals for additional services as needed. Maintain annual national certifications and bi-annual state licensure. Complete required MCMC training per policy, as well as maintains compliance with and knowledge of all MCMC departmental, facility specific and system-wide policy and procedure. Attend mandatory departmental meetings and/or trainings. Follow MCMC’s Mission, Vision, and Values. Functions within the scope of their approved competency skills checklist. Assume other responsibilities as directed, as well as maintains compliance with and knowledge of all MCMC departmental, facility specific and system-wide policy and procedure.

Education/Licensure:

Master’s Degree in Social Work or Counseling Nationally Certified for Applicable Degree Licensed in the State of Louisiana as a Licensed Clinical Social Worker or LPC Current CPR certification

Skills/Experience:

Knowledge of assessing, planning, and implementation of psychosocial services. . Ability to counsel/educate patients and interpret any specific psychosocial needs or psychosocial disorders to professionals and lay groups. Ability to conduct surveys and studies relating to psychosocial status and patterns of population groups as directed. Ability to review and evaluate recent psychosocial health research and interpret it to professionals and patients. Ability to perform short-term counseling and crisis intervention as necessary. Ability to assist in the implementation of social programs. Ability to coordinate with clinical staff in the provision of services to patients Provide referrals for services as needed. Ability to provide in-service presentations to staff as needed or required. Ability to provide community service information as an outreach worker if required. Ability to conduct behavioral health related educational presentations to clinical staff, patients, and patient families.
