We do this through delivering a specialized continuum of outpatient care of 1,000+ units/week to a population of ~5,000 across

Massachusetts:

medication management, in-person and virtual intensive outpatient and partial hospitalization programs (IOP/PHP), transcranial magnetic stimulation (TMS), in-house laboratory services, as well as three levels of housing (i.e. Medicaid reimbursed Co-Occurring Recovery Residences, self-pay-based sober homes, and state-sponsored low threshold housing). team members demonstrate respect to our providers, other front line team members, and patients by holding ourselves to the highest standard of accountability Job description: Square Medical Group is looking for energized individuals who want to join our very supportive team in an in-person Intensive Outpatient Program (IOP) and Partial Hospitalization Program (PHP) based in our Woburn clinic. This is an opportunity to work with a dynamic multidisciplinary team treating adults with co-occurring disorders within a comprehensive service system.

Responsibilities:

Participate in daily milieu management of IOP/PHP Facilitate groups in the IOP and PHP, utilizing evidenced based program curriculum as well as incorporating individual service plan (ISP) goals of clients in treatment. Provide individual counseling to assigned IOP/PHP clients Provide compassionate, nonjudgmental clinical treatment in accordance with the scope of practice. Complete biopsychosocial assessments Coordinate care with collaterals and family members as needed In collaboration with treatment team, provide crisis management for clients, as needed, during program hours. Attend meetings as directed and meet with supervisor to receive supervision and participate in clinical reviews. Provide timely documentation of all services in accordance with SMG standards and regulatory and payer requirements, utilizing electronic formats, as required. Abide by the professional staff code of ethics and maintain a high level of ethical conduct regarding confidentiality, dual-relationships and professional stature. Requirements Masters in Social Work, Mental Health Counseling or related Human Services field. Prior experience in substance use and/or mental health disorder treatment preferred. Prior counseling experience preferred. Prior experience utilizing an Electronic Health Record preferred Knowledge of the substance use treatment continuum and services and related community resources preferred. License/Certification -Licensure of Alcohol and Drug Counselor (LADC I or LADC II) or Certified Addictions Counselor (CAC) or Certified Alcohol and Drug Abuse Counselor (CADAC) preferred Ability to work independently and cooperatively as a member of a professional team.
