Salary:

$80,000 – $110,000 per year A bit about us: Weservice over 30,000 patients throughout the country and over 300skilled nursing facilities! The LCSW would provide evaluation andtreatment of patients with behavioral health issues in long termcare facilities. We provide in-person, telehealth and hybridservices, so there are multiple options within our company.

ESSENTIAL FUNCTIONS

• Maintain a patient caseload – provide diagnostic evaluations,counseling, supportive care, and therapy behavioral management.
• Work as a member of the interdisciplinary team assigned to thefacility.
• Participate in case reviews and consultations.
• Complete patient notes in our Electronic Health Records system.
• Commit to a full day of service at assigned facilities for hourshired to work.
• Participate in staff meetings as appropriate.
