Job Description

The Intake Counselor works closely with the intake team, guiding the patient assessment process by systemically integrating client reports, observations of patient behaviors, results from testing procedures and interactions with patients. The plan for episode of care is initiated at admission to all levels of care.

Shift details:

Part-time 24 hours per week

Schedule:

1st shift – 7:00AM-3:30PM or 2nd shift 3:00PM-11:30PM available

Essential Responsibilities:

+ Plans, coordinates, and evaluates direct and indirect facility assessments and client admissions. + Documents and processes intake calls in accordance with Intake policies. + Directs the screening of potential clients for admission into the program and initiate the integrated assessment process. + Pre-certification is obtained in accordance with insurance requirements. + In house assessments are completed with treatment referral recommendations made and documented; using the approved admission criteria and making decisions concerning the level of care for treatment using the least restrictive level appropriate.

Our state-of-the-art facility is conveniently located in Westborough near the intersection of Route 495 and Route 9. The hospital operates 24/7 with multiple shifts available. Requirements + Master’s Degree along with LCSW, LICSW, or LMHC required. + Minimum of 2 years’ experience working in an inpatient behavioral health environment preferred. + knowledge of patient admission to include insurance coverage, communication with Psychiatrist/Physician, Documentation, Assessment procedures. + Demonstrates knowledge of health status, mental health status, systems involved in the client’s life, further understanding of the clinical need an implication of persons with comorbid disorders.
