Summary:
Intensive In-Home (IIH) service is a team approach to address the identified needs of children and adolescents who, due to serious and chronic symptoms of an emotional, behavioral, or substance use disorder, are unable to remain stable in the community without intensive interventions. This service may only be provided to individuals through the age of 20. This medically necessary service directly addresses the individual’s mental health or substance use disorder diagnostic and clinical needs. The needs are evidenced by the presence of a diagnosable mental, behavioral, or emotional disturbance (as defined by DSM-5, or any subsequent editions of this reference material), with documentation of symptoms and effects reflected in the Comprehensive Clinical Assessment and Person-Centered Plan (PCP). This team provides a variety of clinical rehabilitative interventions available 24 hours per day, 7 days per week, 365 days per year.
The service is intended to:
(1) reduce presenting psychiatric or substance use disorder symptoms, (2) provide first responder intervention to diffuse current crisis, (3) ensure linkage to community services and resources, and (4) prevent out of home placement for the individual.

- Ensures IIH is delivered in accordance with NC DMA Clinical Coverage Policy #8A.
- Provides psychoeducation as indicated in the Person-Centered Plan.
- Consults with identified medical (such as primary care and psychiatric) and non-medical providers (for example, the county department of social services (DSS), school, the Department of Juvenile Justice, and Delinquency Prevention (DJJDP), engages community and natural supports, and includes their input in the person-centered planning process.
- Participates in the person-centered planning process.
- Assists with implementing a home-based behavioral support plan with the youth and his or her caregivers as indicated in the Person-Centered Plan.
- Assists the team leader in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person-Centered Plan.
- Participates in ongoing fidelity monitoring through a combination of internal and external monitoring of EBP fidelity.

- Assists with crisis interventions.
- Assists the team leader in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process.
- Coordinates and oversees the initial and ongoing assessment activities.
- Convenes the Child and Family Team for person-centered planning.
- Provides intensive case management which includes the following: 1. Assessment; 2. Planning; 3. Linkage and referral to paid and natural supports; and 4. Monitoring and follow-up
- Completes the initial development and ongoing revision of the PCP and ensures its implementation.
- Ensures linkage to the most clinically appropriate and effective services including arranging for psychological and psychiatric evaluations.
- Provides and coordinates behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team members.
- Monitors and documents the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the PCP.
- Generates appropriate documentation of services provided and submits them in a timely manner in the Checkpoint Integrity Support Electronic Health Record (EHR) system, as stipulated in the organization’s policy. Utilizes the chosen Evidence Based Practice model (Ecosystemic Structural Family Therapy (ESFT)) to all IIH clients.
- Attends required meetings (i.e., monthly supervisions, staff meetings, weekly IIH Team monthly ESFT case concepts, semi-annual ESFT boosters).
- Attends required and recommended training(s) to keep abreast of agency, MCO and Division’s regulations, guidelines, and procedures.
- Ensures incident reports are accurately documented and reported in IRIS within required timelines.
- Participates in client chart audits and other QA/QI projects.
- Performs other duties, tasks and assignments delegated by supervisor.

Other Position Characteristics Accuracy of work by the individual in this position will have a tremendous impact on treatment outcomes for clients and parents/families. Efficient allocation of fiscal resources is mandatory in supporting and realizing the primary goals and objectives of children/adolescent services. This accuracy requires visual/perceptual, verbal, and written communication skills. Consequences of error can range from inconsequential to severe. Knowledge of organization’s guidelines, policies, and professional standards for treatment, Managed Care Organizations (MCOs) and NC DHHS regulations is required. Regular contact with organization’s staff members; contact with parents/guardians, community agencies, school juvenile justice and MCOs. There is minimum physical effort to fulfill the requirements of this position. Basic office equipment, including personal computer and materials used in the community based therapeutic practices. Visual observations of supervisory staff’s interactions with clients and their natural supports are essential for ongoing evaluation purposes. Visual observation is also essential to the client’s treatment and proper allocation of resources. Intense mental concentration is necessary for the following: managing a client through crisis, managing staff retention and training, overall supervision of staff, fulfilling medical record requirements and other essential documentation, and evaluating potential emergency placements, etc. All possible safety measures will be met to protect clients and other staff if the need should arise. Experience Qualified Professional (QP) who has the knowledge, skills and abilities required by the population and age to be served. Individual shall have a minimum of one (1) year of documented experience with the child/adolescent MH/SA population. Specific Skills Required
