We are seeking a dedicated and compassionate Qualified Professional to join our Community Support Teams. The ideal candidate will provide essential support and services to individuals with mental health challenges, facilitating their recovery and enhancing their quality of life in the community.   • Key Responsibilities:

  • Planning:
  • Write and develop individualized Person-Centered Plans (PCP) that reflect clients’ needs, strengths, and preferences.
  • Collaborate with clients, families, and multidisciplinary teams to ensure plans are comprehensive and aligned with the North Carolina Clinical Coverage Policy guidelines. 2.
  • Service Delivery:
  • Provide direct support and interventions, including crisis intervention, skill-building, and therapeutic support.
  • Facilitate access to community resources, services, and support networks.
  • Monitor and document clients’ progress, adjusting support plans as necessary. 3.
  • Collaboration and Coordination:
  • Work collaboratively with other professionals, including psychiatrists, social workers, and case managers, to ensure holistic care.
  • Participate in team meetings and case consultations to discuss client progress and strategies for improvement.
  • Advocate for clients’ needs within the community and healthcare systems. 4.
  • Training and Development:
