Qualified Professional/Case Manager 3.8 3.8 out of 5 stars Whiteville, NC 28472 Job Summary Integrated Solutions of NC is seeking a dedicated and compassionate Qualified Professional (QP) to join our Community Support Team (CST). The QP will play a critical role in providing therapeutic interventions, care coordination, and support services to individuals experiencing mental health and substance use challenges. This position requires leadership, clinical expertise, and a commitment to improving the lives of the individuals we serve.
Duties Assessment & Planning:
Conduct comprehensive assessments to identify client needs, strengths, and goals. Develop and implement person-centered treatment plans.
Direct Support:
Provide therapeutic interventions, crisis stabilization, and skill-building to clients in various settings, including the community, home, and office.
Care Coordination:
Collaborate with other service providers, healthcare professionals, and community resources to ensure seamless care for clients.
Documentation:
Maintain accurate and timely documentation of all services and interactions in accordance with regulatory and organizational standards.
Team Collaboration:
Work closely with CST team members, including the team leader, peer support specialists, and other QPs, to deliver cohesive and effective services.
Advocacy:
Advocate for clients’ needs, ensuring access to appropriate services and resources to support their recovery and overall well-being.
Compliance:
Adhere to all organizational policies, state regulations, and ethical standards in behavioral health care.
Qualifications:
Bachelor’s degree in a human services-related field (e.g., psychology, social work, counseling) with at least 2 years of experience working with individuals with mental health or substance use challenges; OR Master’s degree in a related field with 1 year of experience. Current licensure or certification as a QP in North Carolina. Strong knowledge of evidence-based practices in mental health and substance use treatment. Excellent communication, organizational, and interpersonal skills. Proficiency in documenting services and working with electronic health record systems.
