Full Job Description Description:

This is a HYBRID position – All visits with clients in person; some in-person meetings; some remote work, completing paperwork. An IIH-QP is a member of the treatment team supporting the client and the development and implementation of the client’s goals identified in the person-centered plan (PCP). The IIH-QP works collaboratively to ensure that the client’s therapeutic needs are addressed, providing intensive services and supports to reduce symptoms and improve skills so the client can function effectively in the home and in community and in educational environments. Therapeutic activities offered are designed to assist with the development of living skills and self-management and behavioral management skills in areas of need related to but not limited to anger-management, social skills, communication enhancement, and problem-solving skills to assist the client in developing of positive behavioral outcomes that will enable the client to maintain appropriate functioning in community settings, the home, as well as in educational environments. The IIH-QP plays their role in the IIH team, providing first responder crisis response, as indicated in the PCP, 24-hours-a-day, 7 days-a-week, 365 days-a-year. Develop and implement the client’s PCP collaboratively with the client and the treatment team according to client’s therapeutic needs, ensuring they are addressed. Provide intensive services for the purpose of reducing symptoms and improving function so the client can maintain appropriate behavior in home, community, and education settings. Nurture supportive and therapeutic relationships with the client, caregivers, and other providers assisting the client and those in the home/family. Monitor psychiatric symptoms and self-management behavior/symptoms, encouraging positive behavior and offering supportive, therapeutic interventions/activities to reinforce positive outcomes. Engage with the client during each session, delivering the required number of billable contact hours at the appropriate frequency, according to the enhanced service definition in

Clinical Coverage Policy No:

8A. Maintain timely communication with the IIH therapist, implementing clinical interventions, services, and activities to support the client’s growth in behavioral and skills development. Ensure accuracy and timely submission of all service notes, according to Agency timeframes. Assist with case management and linking activities to ensure that the client obtains needed services and supports, monitoring and assessing natural supports as well as progress and regression and reporting to appropriate service providers as needed and reporting any incidents according to Agency policy. Assist with transitions and planned or unplanned discharges, working to maintain client rights, health, and safety as well as therapeutic relationships. Coordinate with all members of the IIH team to guarantee 24/7/365 coverage and first responder support, according to the enhanced service definition in

Clinical Coverage Policy No:

8A. Cooperate with all other involved agencies, services, and programs in order to provide the highest level of service and to attend to the needs of the Agency. Follow CTS Health policies and procedures as well as the Code of Conduct. Maintain confidentiality according to policies and procedures. Attend team planning and training meetings and scheduled supervision and annual performance reviews. Follow chain-of-command when encountering problems or other areas of need. Monitor the safety of self and clients according to policies while also reporting all incidents, issues, and concerns to the NC Community Services Director by the close of the workday in which they occur. Complete other duties as assigned by personnel in authority and/or in CTS Health chain-of-command as needed.

through experience served with the specific population and with a bachelor’s degree in a human services field or other acceptable degrees and combinations of experience with population serviced.
