Full Job Description Position Title:

Community Support Team Qualified Professional (CST-QP)

Objective:

No Bounds Care of NC is looking for a qualified and dedicated Community Support Team Qualified Professional (CST-QP) with experience in community-based services and at least two years of work with the population we serve. The ideal candidate will have strong case management abilities, excellent organizational skills, a solid work ethic, effective communication skills, and proficiency in writing. The CST-QP will provide treatment and support for consumers enrolled in the Community Support Team program, working in both home and community settings. This role reports directly to the Community Support Team Lead.

Key Responsibilities:

Coordinate and oversee both initial and ongoing assessment activities for consumers. Develop, review, and ensure the implementation of Person-Centered Plans (PCPs) for each consumer. Collaborate with medical (e.g., primary care, psychiatric) and non-medical providers, as well as community and natural supports, to integrate their input into the planning process. Facilitate connections to appropriate and effective clinical services, including arranging psychological and psychiatric evaluations when necessary. Provide and coordinate behavioral health services and interventions for individuals and their family members, in partnership with other licensed professionals. Monitor and document the status and progress of all assigned individuals.

Education & Experience Requirements:

A Master’s degree in a human service field, with at least 1 year of full-time post-graduate experience in mental health, developmental disabilities, or substance abuse counseling with the population served, OR A Bachelor’s degree in a human service field, with at least 2 years of full-time post-bachelor’s experience in mental health, developmental disabilities, or substance abuse counseling with the population served, OR A Bachelor’s degree in a non-human service field, with at least 4 years of full-time post-bachelor’s experience in mental health, developmental disabilities, or substance abuse counseling with the population served. Valid driver’s license, auto insurance, and reliable transportation. Strong clinical knowledge of the treatment and service needs of the client population. Skills in assessing client strengths and needs, developing treatment plans, and implementing clinical services.
