JOB SUMMARY:
Provides team-based behavioral health consultation, care coordination and population management to individuals (patients and/or employees) and families in order to improve psychosocial functioning and overall wellness. Behavioral health services may be performed in person or through telehealth in a variety of settings including primary care and specialty clinics, workplace settings and othermunity locations.
EDUCATION/EXPERIENCE
Master’s degree in Counseling, Social Work, or Psychotherapy from an accredited institution. Minimum of 2 years of clinical behavioral health experience. Theological education and/or experience in providing spiritually integrated behavioral health services highly valued.
LICENSURE, CERTIFICATION, and/or
REGISTRATION
Professional behavioral health licensure (i.e.
LCSW, LCMHC, LMFT, NCFBPPC, LCAS
) in North Carolina or applicable state.
ESSENTIAL FUNCTIONS
Provides high quality, spiritually integrated, and trauma informed behavioral health services to individuals and families in collaboration with primary care/wellness team.
Delivers brief behavioral health interventions and works with care team to connect individuals to higher levels of care when needed.
Evaluates crisis situations and responds with appropriate interventions.
Maintains accurate and timely clinical documentation.
Contributes to departmental effectiveness and operations. Actively participates in department meetings, staff development and case reviews. Represents and promotes the department in themunity.
Provides education related to mental health and substance use, along with available information on treatment options.
Provides training on mental health, wellness, and resiliency.
In workplace settings, collaborates with employer to identify areas wanization can promote trauma informed practices, wellness, and resiliency.

SKILLS/QUALIFICATIONS
Excellent working knowledge of behavioral health and evidence-based treatments for behavioral health conditions.
Familiarity with Electronic Medical Records and/or proficiency in basicputer skills and willingness to learn.
Comfort working with interdisciplinary team.
Strong oral and writtenmunication skills.
