As our Assertive Engagement QP, you will provide direct care services to identified clients that are appropriate for Assertive Engagement services. The position provides a short-term engagement targeted to populations or specific consumer circumstances that prevent the individual from fully participating in needed care for mental health or addiction issues. You will play a critical role in addressing Social Determinants of Health needs and any other barriers to allow a smoother transition into ongoing services through flexibility and meeting the consumers’ particular needs in their own environment or current location (i.e., hospitals, jail, shelters, streets, etc.) as they transition into the appropriate level of care. This is a community-based role that covers multiple counties – we provide mileage reimbursement! Pay Starting pay for this role is $21.00 per hour. Schedule Monday – Friday, flexible daytime hours. You would receive 10 paid holidays as well as PTO hours which would accrue at a rate of 10 hours per month. After 6 months you would be eligible to enroll in our 401(k) plan which has a 4% match. Job Responsibilities of an Assertive Engagement Qualified Professional (QP) Provide case management services as necessary to clients on caseload Participate in outreach as necessary to build caseload in the community Establish relationships with a comprehensive, targeted listed of providers and other relevant community organizations including Meet with clients in the community, homes, and offices as needed (remain within the location ratio specified in service definition) Assist with addressing barriers to engaging in services in treatment settings Prevent and mitigate housing crises including being a point of contact for landlord Assist in developing daily living skills to stabilize and maintain housing Complete thorough transition plans for each client and assure a smooth transition to the next level of care Participate in supervision (monthly minimum) and necessary treatment team meetings Qualifications of an Assertive Engagement Qualified Professional (QP) Education and Experience Two years’ experience working directly with adults struggling with mental health and substance use disorders Two years of experience working in community mental health setting preferred QP or AP status required as specified in NC statute – QP preferred Knowledge, Skills, and Abilities Ability to easily connect with people with profound communication skills A constructive approach towards members Ability to manage crisis situations both in person and over the phone Ability to maintain confidentiality of the information collected from the members and their families Considerable knowledge of the dynamics and problems of addiction and current practices of treatment Strong commitment to the rights and abilities of each person with a severe and persistent mental illness to live in normal community residences About SouthLight Healthcare Tap into your passion at SouthLight Healthcare.
