We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.
The Family Support Manager will supervise and oversee the Family Support Department under the oversight of the Vice President of Strategic Alliances and Partnerships. Family Support staff assist individuals/families/guardians through education and support to develop and achieve the individual’s personal visions. This is accomplished by sharing information and making referrals to various local, state, and federal resources through collaboration and advocacy. Family Support works to link individuals/families/guardians to education and training opportunities and to develop short and long-term person-centered planning to identify goals and initiate actions to make those goals a reality. The Family Support Manager directly assists and supports the Family Support Specialists and Family Support Coordinator to perform their duties and responsibilities to meet the above identified mission. Staff within the Family Support department provide resource and referral support to individuals and families, both with and without Medicaid coverage. The Family Support Manager will occasionally (when necessary) carry out the primary responsibilities of the Family Support Specialist for a reduced number of individuals, as well as provides orientation, technical assistance, on-going training and supervision of Family Support staff members.

Qualifications:
Education:

Bachelor’s Degree in Human Services or related field preferred.

Experience:

Four years’ experience in human / developmental services, special education, or a related field preferred. Managerial or Supervisory experience preferred.
Reliable transportation, a valid driver’s license, and proof of automobile insurance.

Compensation and Benefits:

There is a $1000 sign-on bonus for this position – $500 will be paid at the time of hire and $500 will be paid after 6 months in the role.
•Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.

Paid Time Off:

For a full-time employee working 40 hours a week, 22 days of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

Specific Duties and Responsibilities:

Ensures that Family Support Specialists are working with individuals to assist in the identification of natural supports, determine current unmet needs, and future plans. Will assist Family Support staff to engage individuals/families/guardians in the process of person-centered-planning to clarify the individual’s personal values, preferences and vision for the future.
Effectively creates, implements, and manages projects for the Family Support team. Works with staff to ensure the timely completion of assigned projects within timeframes, and meets required deadlines.
Oversees the Developmental Disabilities Waiver Respite service, authorizations, and tracks utilization.
Assist individuals/families/guardians to identify future support needs, including maintaining waitlist profiles and waiting list contacts, clearly listing services needed but not currently available and listing services to be provided without reference to program models. Arranges, as requested and necessary, ancillary services including the necessary documentation to obtain appropriate program funding and provide the necessary supporting documentation to internal and external partners.
Oversees the Family Support Coordinator role and collaborates on up-to-date, accurate information and knowledge of community resources, information, and referral. Works with the Family Support Coordinator to ensure this information is available to staff and the families/individuals supported through the Area Agency.
Assists Family Support Specialists to facilitate Basic Service Agreement development, including coordinating the meeting, determining responsibilities for providing services, assuring that goals are related to individual’s desired future, and ensuring completion of necessary evaluations/assessments on behalf of the individual.
Performs all duties in a manner that demonstrates an awareness of the respect for each individual’s legal, civil, and human rights-including maintaining confidentiality of information regarding the individual and family that ensures the respect and dignity of persons with a developmental disability. Provides all support and advocacy in a manner that takes into consideration the preferences of individuals/families/guardians which actively encourage choice making and self-determination by the family and individual.
Meets with VP of Strategic Alliances and Partnerships, as needed to review current information around families and FS staff.
Attends trainings and seminars as required.
Provides orientation and training to new Family Support Staff.
Follows all applicable program policies and procedures of Community Bridges during the performance of these job duties.
Ensures compliance with applicable state regulations.
Develops plans for transitional phase out of support involvement as appropriate, with the individual/family/guardian.
Collaborates with internal Fiscal Committee as needed.
Performs other general administrative duties including the record keeping of individual files to include written progress notes, updating NH Easy information, attend state meetings and participating in agency committees.

Supervisory Responsibilities:

Supervises and mentors a team of Family Support Specialists and the Family Support Coordinator.
Provides internal onboarding and training of Family Support staff regarding person-centered planning, facilitation and service design technique, as well as processes and tools.
Completes annual performance evaluations for Family Support staff.
Coordinates professional development and training opportunities to be offered to Case Managers and Family Support staff, as well as individuals/families/guardians with regard to person-centered-future planning.
Works with Family Support staff to ensure the timely completion of assigned projects within timeframes and meets requested deadlines.
Assist Family Support staff to design planning processes to implement with individuals, families and guardians.
Completes projects that align with strategic goals, as well as ensures Family Support meets regulatory components.
Collaborates with other Individual and Family Support Department Directors to ensure staff uses best practices and provide quality support to eligible individuals.

Required Skills:

Familiarity of and experience with implementing various person-centered-planning techniques, processes and practices.
Ability to research additional innovative person-centered-planning models and practices and assist Individual and Family Support staff to learn and implement new practices where appropriate and effective.
Possess knowledge of Medicaid and other public benefits (and the application process) that are accessible to some individuals within Family Support.
Ability to design group trainings and to custom tailor staff development methods to the needs and styles of each staff supervised and the department as a whole.
Acts as an agency representative in reviewing and signing off on Basic Service Agreements or other documents.
Works to develop, monitor and evaluate the efficacy of departmental procedures/practices to ensure a seamless process for service provision.
Participates in statewide Family Support meetings and committees.
Ability to communicate thoughts and ideas clearly and effectively in both verbal and written formats, and in a group or one-to-one settings.
Ability to advocate for, and provide support and assistance to individuals/families/guardians and advocate for their rights.
Ability to represent Community Bridges’ services to the public and maintain a positive image to the community at large.
Ability to work effectively with staff as assigned to supervise; the greater department and the ability to carry out initiatives in accordance with the departments mission, goals and objectives.
Ability to develop positive collaborative relationships with individuals/families/guardians and their team members and community citizens on behalf of each individual/family/guardian.
Maintain on call responsibilities as necessary.
Must be able to physically conduct home visits, with or without accommodations.
Must possess knowledge of supervisory practices and group process skills as well as the ability to facilitate large groups.
Working knowledge of behavioral strategies.
Working knowledge of supports and services for individuals who may possess psychiatric disorders and acquired brain disorder and their impact.
Be familiar with the functions and regulations of ancillary support systems.

Work Environment and Physical Demands:

Can be occasionally hectic with occasional periods of high stress. Occasional short deadlines and coverage require flexibility in schedule.
Travel required within the state frequently, with occasional out of state travel. Must have access to personal vehicle during all scheduled work hours, maintain valid driver’s license and appropriate insurance, and uphold an acceptable driving record. May be required to transport individuals and families receiving services on occasion.
