Intake Counselor- Addictions Program UConnectCare Behavioral Health Services, Inc. A full time position is available with addictions program in Batavia, Genesee County, NY.

Duties:

The Intake Counselor will be responsible for gathering pertinent information relevant to the client’s evaluation, through the use of diagnostic tools, interviews, collateral contacts, urine screens and information from previous treatment providers. The intake counselor will be responsible for pertinent paperwork associated with the intake to include; releases, fee contract, medical assessment, rights and responsibilities, case notes, diagnostic intake form, MSE, 5 axis, activities waiver, narrative summary, letters to referral source. The intake counselor will be responsible for presenting the clients evaluation to the treatment team during case conference, completing the case conference form and informing the client of the results of case conference, through phone contact or a face to face follow up session. The intake counselor will schedule client with primary treatment counselor following client’s acceptance of treatment recommendation. If client refuses treatment recommendation, client’s record will be screened out. The intake counselor will be responsible for coordinate with DSS, insurance, and other funding providers. The intake counselor will schedule admissions and discharges across the programs to ensure bed fill capacity goals are met in a timely fashion. The intake counselor may be asked to assist in other tasks not limited above.

Reporting Relationship :

The Intake Counselor will report to and receives supervision from the Services Director or his/her designee.

Qualifications :

Candidates must have a minimum of two years’ experience in chemical dependency counseling at an OASAS licensed program and must be familiar with OASAS guidelines pertaining to the evaluation mode. Candidates must have thorough knowledge of the DSM V criteria for Substance Use Disorder. Candidate’s must also be a Qualified Health Professional in the state of New York and/or be willing to obtain CASAC in a timely fashion upon hire. Pre-employment background check and post-offer drug screening required. Applicant must be free of active addiction.
