Job Description

Part-Time Evening Residential Counselor 3.0 3.0 out of 5 stars Hewlett, NY 11557 South Shore Association for Independent Living, Inc. (SAIL) Provides rehabilitation, clinical treatment, and supportive services to the high need, seriously and persistently mentally ill adult population of Nassau County, Suffolk County and NYC-Queens. This is provided by the Community Residence Programs, Apartment Treatment Programs, and Supported Housing Programs (for transitional and permanent residential treatment), and by the Assertive Community Treatment Program (for the non- traditional mental health services). The focus of the treatment and support is to increase consumer’s independence, choices, and outcomes. General Description Responsible for supportive counseling and providing training in independent living skills. Qualification and Competencies An Associates degree and five years experience in the Human Service field or Bachelor’s degree in human service field Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families and the community. Critical thinking and sound judgment skills are necessary. Duties and Responsibilities Participates in the planning and evaluations of individual service plans and monitors plan objectives. Write progress notes on service plan on a monthly basis or more often as needed. Trains and assists residents in the activities of daily living. Monitors medication. Responsible for accompanying residents and providing needed transportation for recreational and social activities. Provide counseling, behavioral intervention and crisis management services. Assist in day-to-day management of the community residence. Responsible for administration of residence in the absence of the Senior Counselor. Notifies the appropriate supervisory personnel of any medical, psychiatric, or programmatic emergency. (See Functional Program) Responsible for other related activities. Provides case management and follow-up services. Provides transportation, as needed, for shopping, medical appointments, etc. Learns and adheres to all agency policies and procedures on an ongoing basis. Accepts and follows through on all other duties and responsibilities as assigned. Physical Demand and Working Conditions Adheres to HIPAA Policies & Procedures/Compliance Plan.
