With these health centers, Mount Sinai builds a primary care practice right at the employer’s worksite where traditional fee-for-service payments are replaced by a value-based alternative that enables the clinical team to provide patient-centered care. We are providing this care at our flagship 9,000 square foot state-of-the-art Health Center at Hudson Yards as well as new onsite employee health centers that we are building in partnership with our large employer customers. These Health Centers provide comprehensive, convenient and exclusive care to Hudson Yards area employees, residents and their families. We are recruiting team members to help continue building these next-generation practices. Delivers evidence-based therapy and treatment Conduct individual intake assessments and psychotherapy as needed for evidence based, outcome-oriented treatment through both traditional face-to-face visits as well as non-visit based (e.g. phone, digital messaging, and video chat). Prepare treatment plans with measurable goals, monitor progress using standardized instruments and update interventions based on patient progress. Initiates referrals to specialty services and ensures bi-directional communication with specialist to promote continuity of care. Carries out other appropriate professional duties as may be necessary, including liaison with outside medical consultants, hospitalists, emergency room providers, etc. to ensure continuity of care, timely, appropriate care, and an excellent patient experience. Works collaboratively with other staff to promote team-based care. Supports clinical operation and improvement activities Actively engages in practice innovation and improvement initiatives. Engages in the review of data and clinical workflows of the practice and ensures systems and programs are in place to provide exceptional, safe, and compliant care. Conducts clinical case review and participates in educational sessions to stay current and improve one’s practice of therapy. Supports wellness offerings and marketing efforts Participates in development of wellness and health promotional offerings, including educational events, nutrition initiatives, fitness classes, etc. Supports the planning, operation, and evaluation of wellness activities. Participates in communicating and marketing the practice model to new patients as well as other stakeholders such as clients, patients, and third-party vendors. Duties Conduct comprehensive patient assessments to determine mental health needs and develop individualized treatment plans. Provide behavioral therapy and crisis intervention services to clients facing various challenges. Utilize diagnostic evaluation techniques to identify mental health disorders and recommend appropriate interventions. Collaborate with interdisciplinary teams to ensure holistic care for clients, including referrals to other healthcare professionals as necessary. Maintain accurate documentation of client progress and treatment outcomes in compliance with regulatory standards. Offer Christian counseling as requested, integrating faith-based approaches into therapeutic practices when appropriate. Stay current with developments in the field of social work and behavioral health, including updates on ICD-9 coding for diagnosis documentation. Skills Strong understanding of behavioral health principles and practices. Proficient in social work methodologies, including patient assessment and crisis intervention techniques. Experience with geriatric populations is highly desirable. Ability to conduct thorough diagnostic evaluations and implement effective behavioral therapy strategies. Familiarity with Christian counseling principles is a plus. Excellent communication skills, both verbal and written, for effective interaction with clients and team members. Strong organizational skills to manage multiple cases while maintaining attention to detail in documentation.
