The responsibilities include, support and monitoring of residents, care coordination, individual/group counseling, discharge planning and medication management. The candidate must be available to work an 8-hour shifts – various schedules available both on the 4 PM to 12 AM shift and the overnight shift. Candidates with a CASAC-T or CASAC are preferred. Valid driver’s license preferred. COVID vaccine or willingness to get one required. SALARY dependent on experience.
