Provides clinical mental health services to all agepatients/families in the Institute for Family Health. The scope ofpractice includes but is not limited to diagnostic assessment,counseling, psychotherapy, brief treatment, behavior modification,crisis intervention and mental health consultation. The socialworker will report to the Regional Director and/or Program Directorand work collaboratively with members of the health care teamthroughout the organization, while utilizing independent judgmentin delivering care and services. This position is a learnedprofessional, requiring achievement of specific advancededucational training related to the duties detailed.

REQUIREMENTS

Provide individual, family, and group counseling, crisisintervention and concrete services. Work in close collaborationwith physicians and other health care personnel in patientevaluation and treatment to further their understanding ofsignificant social and emotional factors underlying patients’health problems. Prepares comprehensive bio-psychosocialevaluations, mental status exams, patient histories, treatmentplans and reports. Conduct short- and long-term therapy withappropriate treatment plans, goals, and objectives. May supervisesocial work student interns. Educates staff regarding psychosocialissues related to patient care. Actively identify child welfareissues and coordinate with the Child Protective coordinatorregarding appropriate reporting procedures. Document all patientservices in patients’ medical records, completes encounter formsfor all patient visits. Assure social service compliance withlegal, regulatory and professional standards, including Article 28and JCAHO (where applicable). Participates in social servicequality assurance program. Participates in Social ServiceDepartment meetings and all required staff meetings. Demonstratesthe knowledge and skills necessary to provide care, based onphysical, psychosocial, educational, safety, and related criteria,appropriate to the age of the patients served in assigned areaMasters in Counseling Clinical Psychology from an accreditedUniversity required MSW with New York State Certification requiredNYS licensure as LMSW, LCSW, LMHC, LMFT, PhD/PsyD preferred Strongdiagnostic and clinical skills. Willingness to supervise internsand attend required supervisor courses Demonstrated experienceworking with diverse populations ie: HIV, homeless, mentally ill,elderly, children, adolescents, etc. Healthcare experiencepreferred. Bi-lingual, Fluent Spanish preferred.
