Summary Provides clinical mental health services to all agepatients/families in the Institute for Family Health. The scope ofpractice includes but is not limited to diagnostic assessment,counseling, psychotherapy, brief treatment, behavior modification,crisis intervention and mental health consultation. The socialworker will report to the Regional Director and/or Program Directorand work collaboratively with members of the health care teamthroughout the organization, while utilizing independent judgmentin delivering care and services. This position is a learnedprofessional, requiring achievement of specific advancededucational training related to the duties detailed.

REQUIREMENTS

Provide individual, family, and group counseling, crisisintervention and concrete services. Work in close collaborationwith physicians and other health care personnel in patientevaluation and treatment to further their understanding ofsignificant social and emotional factors underlying patients healthproblems. Prepares comprehensive bio-psychosocial evaluations,mental status exams, patient histories, treatment plans andreports. Conduct short and long term therapy with appropriatetreatment plans, goals, and objectives. May supervise social workstudent interns. Educates staff regarding psychosocial issuesrelated to patient care. Actively identify child welfare issues andcoordinate with the Child Protective coordinator regardingappropriate reporting procedures. Document all patient services inpatients medical records, completes encounter forms for all patientvisits. Assure social service compliance with legal, regulatory andprofessional standards, including Article 28 and JCAHO (whereapplicable). Participates in social service quality assuranceprogram. Participates in Social Service Department meetings and allrequired staff meetings. Demonstrates the knowledge and skillsnecessary to provide care, based on physical, psychosocial,educational, safety, and related criteria, appropriate to the ageof the patients served in assigned area

QUALIFICATIONS

Bilinguallanguage (Spanish/English) skills preferred Masters in CounselingClinical Psychology from an accredited University. MSW with NewYork State Certification NYS licensure as LMSW, LCSW, LMHC, LMFT,PhD/PsyD NYS license if Clinical Psychologist or Psychiatrist CSWif MSW Strong diagnostic and clinical skills. Willingness tosupervise interns and attend required supervisor coursesDemonstrated experience working with diverse populations ie: HIV,homeless, mentally ill, elderly, children, adolescents, etc.

Healthcare experience preferred. Bi-lingual, Fluent Spanishpreferred.
