- will operate in conjunction with OptumBehavioral Care (OBC) to serve as the core member of the hybridin-person/virtual collaborative care team for OBC clients.
- In this cutting-edge team the Therapist will interface betweenpatients, PCPs, and psychiatrists to support the mental health andphysical health care of patients on an assigned patient caseloadfrom OBC clients and will have the support of OBC’s clinical andoperational teams.

Position Highlights & Primary Responsibilities:
Use virtual and in-person modalities to coordinate care with thepatient’s medical provider and, when appropriate, other mentalhealth providers Screen and assess patients for common mental health and substanceabuse disorders Systematically track treatment response and monitor patients forchanges in clinical symptoms and treatment side effects orcomplications Provide brief behavioral interventions using evidence-basedtechniques such as behavioral activation, problem-solvingtreatment, motivational interviewing, or other treatments asappropriate Participate in regularly scheduled caseload consultation with thepsychiatric consultant and communicate resulting treatmentrecommendations to the patient’s medical provider Develop and complete relapse prevention self-management plan withpatients who have achieved their treatment goals and are soon to bedischarged from the caseload

Required Qualifications:
Meet one of the following license requirements for the state of
Tennessee:
Licensed Clinical Social Worker or Licensed Mental HealthCounselor Licensed Professional Counselor
- MHSP Licensed Marriage and Family Therapist Will consider provisional license 3+ years treating patients with behavioral health conditions
- 1+ years working in primary care or other medical behavioralintegrated setting (i.
e., with other provider types) Experience with assessment and using evidence-based psychosocialtreatments and brief behavioral interventions for common mentalhealth disorders (e.g., cognitive behavioral therapy, motivationalinterviewing, problem-solving treatment, behavioral activation Currently eligible to be enrolled as a provider in the Medicareprogram or able to terminate opt-out (if applicable) prior tostarting a new position

Preferred Qualifications:
Experience in the Collaborative Care Model (CoCM) Demonstrated experience being detail-oriented, organized, and haveexceptional follow-up capabilities Proven ability to maintain effective and professional relationshipswith patient and other members of the care team
- Proven ability to work with patients in person as well as bytelephone and video
- Proven solid grasp of technology solutions and tools and ability toadapt
- The salary range for this role is $59,500 to $116,600 annuallybased on full-time employment.
